Menstrual & Smear Records
Record the dates of your last three periods, cycle length, and any symptoms. Bring previous smear test results, colposcopy reports, or biopsy letters if you have them. If your smear is overdue, consider arranging this before your appointment.

Fertility & Pre-conception
If attending for fertility or pre-conception advice, note how long you have been trying to conceive and any previous pregnancy outcomes. Both partners attending the first appointment is ideal. Bring previous semen analysis results if available.

Pre-operative Preparation
If scheduled for laparoscopic or hysteroscopic surgery, fast from midnight the night before as instructed. Arrange transport — you cannot drive after general anaesthesia. Wear loose, comfortable clothing and leave jewellery at home.

Warning Signs & When to Act Urgently

Warning Signs in Pregnancy

First Trimester
Severe vomiting, heavy vaginal bleeding, or severe one-sided pelvic pain may indicate miscarriage or ectopic pregnancy. Seek urgent assessment — an ectopic pregnancy is a surgical emergency.

Second Trimester
Reduced or absent fetal movements, persistent headache, significant facial or hand swelling, leaking fluid, or cervical pressure require prompt assessment. Do not wait for a scheduled appointment.

Third Trimester
Sudden severe headache, visual disturbances, or significant oedema may indicate pre-eclampsia. Any reduction in fetal movements, regular contractions before 37 weeks, or heavy bleeding requires immediate attention.

Post-delivery
Heavy blood loss (soaking more than one pad per hour), high fever, severe perineal pain, or persistent low mood or anxiety after delivery require assessment. Postnatal depression is treatable — do not suffer in silence.

Managing Your Condition

Obstetrics & High Risk Pregnancy
Gestational diabetes is managed through dietary modification, regular blood sugar monitoring, and — when required — medication. A low glycaemic index diet with controlled carbohydrate portions is the cornerstone of management. Most cases resolve completely after delivery. Your baby will be monitored for hypoglycaemia after birth.
Regular blood pressure monitoring, urinalysis for protein, and fetal wellbeing assessment are essential for pre-eclampsia management. Hospital admission may be required if blood pressure is not well controlled. Low-dose aspirin from 12 weeks is recommended for women at high risk of pre-eclampsia.
Begin counting fetal movements from 28 weeks. Your baby should have a consistent pattern of movement each day. If you notice a reduction or change in movements, contact Dr. Joshi's team immediately — do not wait until your next appointment, and do not rely on home dopplers to reassure yourself.
All Rhesus D negative women require Anti-D immunoglobulin injections during pregnancy and after delivery to prevent alloimmunisation, which can affect future pregnancies. Anti-D is given routinely at 28 weeks and again after delivery if the baby is Rhesus D positive.
Attend antenatal classes and discuss your birth preferences with Dr. Joshi in advance. Staying mobile and upright during early labour encourages optimal fetal positioning. Keep an open mind about pain relief — the safest and most comfortable birth for you and your baby is always the priority.
Gynaecological Conditions
Lifestyle modification — particularly weight management and regular exercise — is the single most effective intervention for PCOS. Even a 5–10% weight reduction can restore ovulation and improve hormonal balance. The combined oral contraceptive pill regulates periods; Metformin improves insulin resistance; surgical ovarian drilling can restore fertility when medication fails.
Endometriosis is a long-term condition requiring ongoing management. Pain management, hormonal suppression (combined pill, progesterone, GnRH analogues), and surgery are all options. Laparoscopic excision of endometriosis deposits offers better long-term outcomes than ablation, particularly for deep disease. Mild spotting and a dark brown discharge are normal for up to 4 weeks after colposcopy treatment (LLETZ/LEEP or cold coagulation). Avoid intercourse, tampons, and swimming during this period. Attend your follow-up smear at the interval specified in your letter — this confirms the treatment was successful.
Hormone replacement therapy (HRT) is the most effective treatment for menopausal symptoms and is safe for the majority of women. Modern preparations use body-identical hormones with an excellent safety profile. Non-hormonal options include SSRIs, cognitive behavioural therapy, and lifestyle modification. Most fibroids are asymptomatic and do not require treatment — regular monitoring with ultrasound is appropriate. Symptomatic fibroids causing heavy periods, pelvic pressure, or pain may be treated medically (tranexamic acid, progesterone, GnRH analogues) or surgically.
